Effective clinical practice in a hospital needs current knowledge together with the skills and right attitude; these should be applied continuously. Failure of this system can be due to ignorance or arrogance. We attempted to correct these deficiencies by formulating a set of policies which were enforced from 1962 to 1983. The policies related to the following: intensive care (including asthma, nutrition and organ donation), drug prescribing and resuscitation. Rationale, aims and objectives: Bioethics and professionalism are standard subjects in medical training programmes, and these curricula reflect particular representations of meaning and practice. It is important that these curricula cohere with the actual concerns of practicing clinicians so that students are prepared for real-world practice. We aimed to identify ethical and professional concerns that do not appear to be adequately addressed in standard curricula by comparing ethics curricula with themes that emerged from a qualitative study of medical practitioners. Method: (...) Curriculum analysis: Thirty-two prominent ethics and professionalism curricula were identified through a database search and were analysed thematically. Qualitative study: In-depth, semi-structured interviews were conducted with 20 medical practitioners. Participants were invited to reflect upon their perceptions of the ways in which values matter in their practices and their educational experiences. The themes emerging from the two studies were compared and contrasted. Results: While representations of meaning and value in ethics and professionalism curricula overlap with the preoccupations of practicing clinicians, there are significant aspects of ‘real-world’ clinical practice that are largely ignored. These fell into two broad domains: ‘sociological’ concerns about enculturation, bureaucracy, intra-professional relationships, and public perceptions of medicine; and epistemic concerns about making good decisions, balancing different kinds of knowledge, and practising within the bounds of professional protocols. Conclusions: Our findings support the view that philosophy and sociology should be included in medical school and specialty training curricula. Curricula should be reframed to introduce students to habits of thought that recognize the need for critical reflection on the social processes in which they are embedded, and on the philosophical assumptions that underpin their practice. While doctors generally enjoy considerable status, some believe that this is increasingly threatened by consumerism, managerialism, and competition from other health professions. Research into doctors’ perceptions of the changes occurring in medicine has provided some insights into how they perceive and respond to these changes but has generally failed to distinguish clearly between concerns about “status,” related to the entitlements associated with one’s position in a social hierarchy, and concerns about “respect,” related to being held in high regard for one’s (...) moral qualities. In this article we explore doctors’ perceptions of the degree to which they are respected and their explanations for, and responses to, instances of perceived lack of respect. We conclude that doctors’ concerns about loss of respect need to be clearly distinguished from concerns about loss of status and that medical students need to be prepared for a changing social field in which others’ respect cannot be taken for granted. In this essay I discuss the historical adequacy of Charles Taylor's philosophical history of secularization, as presented in his A Secular Age . I do so by situating it in relation to the contextual historiography of secularization in early modern Europe, with a particular focus on developments in the German Empire. Considering how profoundly conceptions of secularization have been bound to competing religious and political programmes, we must begin our discussion by entertaining the possibility that modern philosophical and historiographic conceptions (...) of secularization might themselves be outcrops of this unfinished competition. Peter Gordon has rightly observed that Taylor's philosophical history of secularization is a Catholic one, and that this is bound up with a specific view of secularization as a theological and ecclesiological “disembedding” of rational subjectivity from its prior embodiment in a sacral body, community , and cosmos. Taylor delivers this history in his “reform master narrative”: that certain fundamental religious and cultural reforms or changes in early modern Europe wrought the secularization responsible for a modern epoch of “unbelief”. Epistemic relativists often appeal to an epistemic incommensurability thesis. One notable example is the position advanced by Wittgenstein in On certainty (1969). However, Ian Hacking’s radical denial of the possibility of objective epistemic reasons for belief poses, we suggest, an even more forceful challenge to mainstream meta-epistemology. Our central objective will be to develop a novel strategy for defusing Hacking’s line of argument. Specifically, we show that the epistemic incommensurability thesis can be resisted even if we grant the very insights (...) that lead Hacking to claim that epistemic reasons are always relative to a style of reasoning. Surprisingly, the key to defusing the argument is to be found in recent mainstream work on the epistemic state of objectual understanding. (shrink)

Each part concludes with a speculative scenario, where one of the projects is imagined thriving in a future where life is now almost extinct. This book gives a critical assessment of key developments in contemporary French philosophy, highlighting the diverse ways in which recent French thought has moved beyond the philosophical positions and arguments which have been widely associated with the terms 'post-structuralism' and 'postmodernism'. These developments are assessed through a close comparative reading of the work of seven contemporary thinkers: Jean-Luc Marion, Jean-Luc Nancy, Bernard Stiegler, Catherine Malabou, Jacques Rancière, Alain Badiou and François Laruelle. The book situates the writing of each philosopher in (...) relation to earlier traditions of French thought. In differing ways, these philosophers decisively distance themselves from the linguistic paradigm which dominated so much twentieth-century thought in order to rethink philosophical conceptions of materiality, worldliness, shared embodied existence and human agency or subjectivity. They thereby open the way for a radical renewal of the claims, possibilities and transformative power of philosophical thinking itself. The theory of rationality has traditionally been concerned with the investigation of the norms of rational thought and behaviour, and with the reasoning pro‐cedures that satisfy them. As a consequence, the investigation of irrationality has largely been restricted to the behaviour or thought that violates these norms. There are, how‐ever, other forms of irrationality. Here we propose that the delusions that occur in schizophrenia constitute a paradigm of irrationality. We examine a leading theory of schizophrenic delusion and propose that some (...) delusions can be traced to a violation of a condition on thought we call egocentricity. We argue that the violation of egocen‐tricity leads to irrational states that cannot be explained by the traditional categories of irrationality and conclude, therefore, that these states belong in a new branch of the theory of irrationality, that of experiential irrationality. Abstract Havi Carel has recently argued that one can be ill and happy. An ill person can ?positively respond? to illness by cultivating ?adaptability? and ?creativity?. I propose that Carel's claim can be augmented by connecting it with virtue ethics. The positive responses which Carel describes are best understood as the cultivation of virtues, and this adds a significant moral aspect to coping with illness. I then defend this claim against two sets of objections and conclude that interpreting Carel's phenomenology (...) of illness within a virtue-ethical framework enriches our understanding of how illness can be edifying. This article explores how the concept of agency in social theory changes when it is conceptualized as a relational rather than an individual phenomenon. It begins with a critique of the structure/agency debate, particularly of how this emerges in the critical realist approach to agency typified by Margaret Archer. It is argued that this approach, and the critical realist version of relational sociology that has grown from it, reify social relations as a third entity to which agents have a cognitive, (...) reflexive relation, playing down the importance of interaction. This upholds the Western moral and political view of agents as autonomous, independent, and reflexive individuals. Instead, the article considers agency from a different theoretical tradition in relational sociology in which agents are always located in manifold social relations. From this, an understanding is created of agents as interactants, ones who are interdependent, vulnerable, intermittently reflexive, possessors of capacities that can only be practised in joint actions, and capable of sensitive responses to others and to the situations of interaction. Instead of agency resting on the reflexive monitoring of action or the reflexive deliberation on structurally defined choices, agency emerges from our emotional relatedness to others as social relations unfold across time and space. This is the first of two papers responding to ‘recent’ commentary on various aspects of hyperplane dependence by several authors. In this paper I focus on the issues of the relations of HD to state reduction and unitary evolution. The authors who’s comments I address here are Maudlin and Myrvold. In the second paper of this set I focus on HD dynamical variables and localizable properties and measurements and address comments of de Koning, Halvorson, Clifton and Wallace. Each paper ends (...) with some reflections on the implications of HD for the ontology of Minkowski space-time. To set the stage for my responses, I begin this paper with some general position statements designed to correct what seem to be widespread and erroneous construals of some of my views. Two central points are argued for in this first paper. First, dynamical evolution occurs not only within foliations of Minkowski space-time. Rather the transition from the physical state of affairs on any one hyperplane to any other, whether the hyperplanes intersect or are parallel, is always an instance of dynamical evolution between them, generated by some active combination of boost-like transformations and/or time-like translations and/or state reductions and ‘reconstructions’. This point gives rise to a generalized conception of a history of dynamical evolution, allowing for the use of parameterized families of hyperplanes that multiply cover some portions of space-time. Nevertheless, and this is the second central point, for any two generalized histories, H and H’, the quantum states for a system on all the hyperplanes of H from the asymptotic past up to some hyperplane, h, determine the quantum states for the system on all the hyperplanes of H’ from the asymptotic past up to any hyperplane, h’, such that h’ lies to the past of all those state reduction regions that lie to the future of all hyperplanes of H that are ‘earlier’ than h. Consideration of these results should defuse concerns that have been voiced about the coherence and consistency of HD. This chapter is concerned with the representation of time and change in classical (i.e., non-quantum) physical theories. One of the main goals of the chapter is to attempt to clarify the nature and scope of the so-called problem of time: a knot of technical and interpretative problems that appear to stand in the way of attempts to quantize general relativity, and which have their roots in the general covariance of that theory. The most natural approach to these questions is via (...) a consideration of more clear cases. So much of the chapter is given over to a discussion of the representation of time and change in other, better understood theories, starting with the most straightforward cases and proceeding through a consideration of cases that lead up to the features of general relativity that are responsible for the problem of time. Moralists tend to have a low opinion of self-interest. It is seen as force that has to be controlled or transcended. This essay tries to get beyond the bifurcation of human motivations into self-interest (which is seen as vicious or non-moral) and concern for others (which is virtuous). It argues that there are some surprising affinities between self-interest and morality. Notably the principal force that checks self-interest is self-interest itself. Consequently, self-interest often coincides with and reinforces the commands of morality (...) and promotes civility and consideration for others. Therefore it provides us with resources for constructing a more humane and civil society. (shrink)
